What Is Organophosphate Poisoning?

The EPA's manual on recognizing and managing pesticide poisonings describes organophosphates as a class of insecticides, several of which are highly toxic, and says they were among the most widely used insecticides until the 21st century. It says they all share a common mechanism, cholinesterase inhibition, and can cause similar symptoms, although there are some differences within the class.

The manual says symptoms of acute organophosphate poisoning develop during or after exposure, within minutes to hours, appearing fastest after inhalation, then after swallowing and finally after skin exposure, and that all the signs are cholinergic, affecting muscarinic, nicotinic and central nervous system receptors. Among the most commonly reported early symptoms are headache, nausea, dizziness and hypersecretion.

Treatment is medical and urgent. The manual says atropine is effective against muscarinic effects but not against nicotinic effects such as muscle weakness, twitching and respiratory depression, and that despite these limits atropine is often a life-saving agent in organophosphate poisonings. It says an oxime can reactivate some of the affected enzyme depending on timing, and that pralidoxime is the only oxime currently approved by the FDA in the United States.
