What Is Tetrodotoxin?
The CDC says pufferfish poisoning occurs after eating fish containing tetrodotoxin, a potent neurotoxin. Potentially toxic fish are distributed widely worldwide and include pufferfish, porcupine fish and ocean sunfish, and the toxin is usually concentrated in the ovaries, liver, intestine and skin.
The CDC says symptoms may develop within 10 minutes of eating toxic fish or be delayed more than 4 hours, and that early symptoms typically include tingling and numbness of the lips and mouth, followed by nausea and dizziness. It says there is no specific antidote for tetrodotoxin, so treatment is designed to limit absorption of the toxin and provide supportive care.
The FDA restricts imports. Its import alert says there is a high fatality rate among those who ingest tetrodotoxin and the toxin cannot be destroyed by cooking or freezing, so importing pufferfish products into the United States is restricted and personal importation is prohibited. It says imported pufferfish remains subject to detention without physical examination in any and all forms, and that shipments offered outside its agreement are not permitted.
